The Current Model Is Broken
Cyber insurance underwriting in 2026 is essentially a checklist. Do you have MFA? Do you have EDR? Do you have a patch management program? Do you conduct annual penetration tests? Do you have an incident response plan? Check the boxes, get a score, get a premium. The more boxes you check, the lower your rate.
This model made sense when there was no alternative. If every organization stores data in plaintext at some point during processing, then the only variables an underwriter can evaluate are how likely it is that an attacker reaches that plaintext. Firewalls reduce likelihood. MFA reduces likelihood. EDR reduces likelihood. Patch management reduces likelihood. Every control is a probability reducer.
But probability reducers have a ceiling. No combination of firewalls, endpoint detection, access controls, and training programs drives breach probability to zero. The best-defended organizations in the world still get breached. SolarWinds had a SOC. Change Healthcare had compliance certifications. Equifax had a security team. The controls reduce frequency, not severity. When the breach happens — and the actuarial tables confirm it eventually will — the claim is just as catastrophic whether the organization checked 40 boxes or 50.
This is why premiums remain high despite massive investment in security tooling. Underwriters know that the likelihood of a claim dropped as MFA and EDR became standard. But the severity of claims hasn't changed, because the thing that drives severity — plaintext data exposure — hasn't changed. The average breach still costs $4.88 million. Healthcare breaches still average $9.77 million. The floor hasn't moved because the architecture hasn't moved.
Breach Probability vs. Breach Yield
Here's the distinction that changes everything:
Breach probability is the likelihood that an attacker gains unauthorized access to a system. This is what current underwriting models price against. It's influenced by MFA, EDR, patching, training, network segmentation, and every other preventive control.
Breach yield is the value of what the attacker obtains when they succeed. This is what actually determines claim severity. It's influenced by one thing: whether the data the attacker reaches is readable.
Current models treat every breach as roughly equivalent in yield. If an attacker reaches your database, they get records. The claim size scales with the number of records and their sensitivity. This assumption is baked into every actuarial table in the industry.
But what if the attacker reaches the database and the records are encrypted with keys that don't exist on the server? What if they compromise the application server and the data in memory is ciphertext? What if they exfiltrate everything they can find and all of it is mathematically unreadable without a key that never left the customer's device?
The breach probability might be identical. The breach yield is zero.
That's not incremental risk reduction. That's a different actuarial category entirely.
What Zero Yield Looks Like Technically
Fully homomorphic encryption makes it possible to process data without ever decrypting it. The server performs computations — searches, analytics, machine learning inference, biometric matching — on ciphertext. The results return encrypted. The server never possesses the decryption key.
This isn't encryption at rest (which requires decryption for use). It isn't encryption in transit (which protects the pipe but not the endpoints). It's encryption during processing — the phase where every traditional breach extracts plaintext. The memory scraper finds ciphertext. The SQL injection returns ciphertext. The compromised admin account sees ciphertext. The insider threat accesses ciphertext. Every attack vector that currently produces a multimillion-dollar claim produces... nothing.
This is running in production today. H33 processes 2.17 million encrypted authentications per second on a single ARM server. Each authentication includes FHE biometric matching, STARK zero-knowledge proofs, and CRYSTALS-Dilithium post-quantum signatures. No decryption at any step. 38.5 microseconds per operation. The performance overhead that made FHE impractical five years ago is gone.
The Actuarial Implication
Consider how a breach-yield model changes the math for an underwriter evaluating two otherwise identical organizations:

Same breach probability. Same attacker. Same initial compromise. But the expected annual loss drops from $146,400 to $7,950 — an 18x reduction. Not because the perimeter is better, but because the yield is zero.
An underwriter pricing Organization B the same as Organization A is mispricing risk by a factor of 18. The carrier that figures this out first writes the most profitable book of cyber business in the market.
Why This Hasn't Happened Yet
Three reasons:
1. FHE wasn't practical until recently. Five years ago, a single FHE operation took seconds. You couldn't run a production application on it. The underwriting models were built in an era where encrypted processing was academic. That era ended. We're at 2.17 million operations per second. The tech is ahead of the actuarial models.
2. Underwriters don't have a framework for evaluating cryptographic architecture. They have questionnaires for firewalls, MFA, EDR, and patching. They don't have a questionnaire that asks "does your application server ever possess plaintext customer data?" They don't have a checkbox for "are your AI inference endpoints FHE-wrapped?" The evaluation tools haven't caught up. HATS certification is designed to fill this gap — a single credential that tells an underwriter the data is cryptographically protected at every layer.
3. The industry hasn't seen enough zero-yield breaches to update the models. Actuarial tables are backward-looking. They're built on historical claims data. There isn't enough historical data on organizations using FHE because the technology just reached production speeds. This is a chicken-and-egg problem: carriers won't update models until they see data, and they won't see data until policyholders adopt FHE. The carrier that breaks this cycle by proactively pricing FHE-protected organizations differently will attract the best risks in the market.
The Harvest-Now-Decrypt-Later Time Bomb
There's an additional dimension that makes breach-yield pricing urgent: quantum computing.
Nation-states are recording encrypted traffic today with the explicit intention of decrypting it when quantum computers mature. This means every policy written today carries a latent liability that won't materialize for 5-15 years. An underwriter writing a 2026 policy for an organization using RSA-2048 key exchange is unknowingly underwriting a future quantum decryption claim. The data was encrypted in transit, yes — but the encryption will be breakable, and the data was recorded.
Under a breach-probability model, this risk is invisible. The organization wasn't "breached" in any traditional sense. The traffic was intercepted passively. The claim materializes years after the policy expires.
Under a breach-yield model, the question becomes: if the traffic is eventually decrypted, what does the attacker get? If the organization was using FHE — where the data inside the TLS session is also encrypted with lattice-based homomorphic encryption that is inherently quantum-resistant — the yield is still zero. Breaking the TLS layer gives you... more ciphertext. The HNDL attack fails not because the recording was prevented, but because the recording is worthless.
CISA has formally determined that post-quantum cryptographic products are "widely available" in specific categories. This establishes a legal standard of care. Organizations that suffer a future quantum-enabled breach after CISA's determination may face the argument that they failed to adopt available protections. That's a liability claim that current policies are pricing at zero.
What Smart Carriers Will Do
The carriers that move first will:
- Add cryptographic architecture questions to their applications. Not just "do you encrypt data at rest?" but "is data ever decrypted during processing?" and "does your application server possess decryption keys for customer data?"
- Create a separate risk tier for zero-yield organizations. Organizations that can demonstrate FHE-protected data processing should be in a fundamentally different actuarial pool than organizations that decrypt for processing.
- Accept HATS certification as a premium modifier. A single credential that verifies encrypted inference, post-quantum readiness, device attestation, and data provenance — evaluated through cryptographic proof, not documentation review.
- Price HNDL exposure explicitly. Ask about post-quantum key exchange. Ask about traffic recording mitigation. Price the latent quantum liability instead of ignoring it.
- Offer premium reductions that exceed the cost of implementation. If FHE reduces expected loss by 18x and costs $9,143/year to implement, the carrier can offer a $50,000 premium reduction and still improve their loss ratio. The policyholder saves money. The carrier writes better risk. Everyone wins.
The Competitive Moat
This isn't just about individual policy pricing. It's about portfolio composition.
A carrier that preferentially attracts zero-yield organizations builds a book of business with fundamentally lower loss ratios. Their claims costs drop not because they selected healthier risks on a probability basis, but because the claims that do occur cost 18x less. They can underprice competitors on premium while maintaining better margins. They attract more zero-yield organizations, further improving the portfolio. The cycle compounds.
Meanwhile, carriers still pricing on probability alone are left with the organizations that haven't adopted cryptographic protection — the organizations most likely to produce catastrophic claims. Adverse selection, driven not by risk appetite but by actuarial model sophistication.
The carrier that prices on yield will outperform the carrier that prices on probability. Not by a little. By the entire distance between a $4.88 million claim and a $265,000 incident response bill.
The Favor to Their Clients
Here's the part that makes this more than a business strategy: carriers that mandate cryptographic protection as a condition of coverage don't just write better policies. They force their policyholders to adopt architecture that makes breaches structurally harmless.
This is exactly what happened with MFA. Carriers mandated it starting in 2023. By 2025, MFA adoption was near-universal among insured organizations. Not because a regulation required it. Not because a CISO convinced the board. Because the insurance company said "no MFA, no policy." The financial incentive was immediate and unambiguous. Carriers drove more security adoption in two years than a decade of NIST guidelines and compliance frameworks.
The same dynamic will play out with post-quantum authentication and encrypted processing. When a carrier says "we'll cut your premium 25% if you implement FHE on your customer data processing pipeline," the CFO approves it in the same meeting. When the implementation is a single API integration that takes days instead of months, the barrier evaporates.
The carriers smart enough to price against cryptographic proof rather than policy compliance will own the next decade of cyber insurance. And every organization they insure will be structurally safer — not because they wrote better policies, but because the architecture makes the policies unnecessary.
The Bottom Line
Right now, premiums reflect breach probability. The better model prices against breach yield.
If the data is never decrypted during processing, the yield is nothing. The attack still happens. The exfiltration still happens. The attacker gets ciphertext they will never, ever be able to read — not today, not with quantum computers, not ever.
That's not incremental risk reduction. That's a different actuarial category entirely.
The question for every carrier is simple: do you want to keep pricing against the lock on the door? Or do you want to price against the fact that there's nothing inside worth stealing?
